Northwest High School Registrar's Office maintains records of former and current NHS students. Former students or their parents/guardians (if the student is under 18) may obtain a copy of their records in person or send in a written request.

Attached below is the Transcript Request form. Your request will be processed when the fee for the transcript is received.  Any fines/fees owed to the school must be paid before the transcripts are released.

Colleges generally request a copy of your immunization as well.  Please specify on your request if you would like us to include immunization.

ACT or SAT scores will be sent with the transcript if those scores are on file at Northwest High School.

Enclose $3 for each request.  Credit/debit cards are not accepted.  Mail your request along with your cash, check or money order made payable to Northwest High School to:
